The A/TO ratio needs to end. The logic is flawed and makes carelessness excusable. Getting assists does not require risky passes and risky passes should be analyzed on a probability basis. If the player is doing the analysis properly he will have very few turnovers but still have plenty of assists. Team assists would be higher but individual assists could be lower.
I think the other aspect of that ratio is that those with more assists likely have the ball in their hands longer and therefore will have more turnovers. That is generally true but you can better base this on turnovers per minute the player holds the ball.
The key to turnovers is that players should be motivated to get the ball back on the defensive end. You don't offset a turnover with an assist, you offset it with a steal, block or rebound. Only the steal or forcing a turnover is a direct offset but the others are preventing points or extending possessions.

I asked a ref if he could give me a technical foul for thinking bad things about him. He said, of course not. I said, well, I think you stink. And he gave me a technical. You can't trust em.

If the pass is missed then I assume a TO for the passer which may or may not be his fault. If the pass is fumbled I would think it is a TO for the recipient but I'm not aware of the official rule. In reality it should be a subjective call because the pass could put the player in an unreasonable position to catch it but could also be a blown play by the "receiver".
